For those seeking consistent, hands-on support, our Weekly Check-Ins with Appointment Management offer a structured and proactive approach to ongoing care. This premium add-on provides up to 8 hours of support per month, ensuring reliable guidance, planning, and peace of mind.

What’s Included:

✔️ Two in-person visits per month

✔️ Two virtual sessions per month

✔️ Total of 8 hours of support monthly

✔️ Appointment management, including scheduling, preparation, and follow-up for healthcare or service visits

✔️ Progress check-ins and plan adjustments based on evolving needs

✔️ Answers to ongoing or emerging questions and concerns

✔️ Two sessions per month can be used for Family Dynamic Support, including:

• • Navigating differing family goals
  • Providing updates to Powers of Attorney (POAs)
  • Facilitating open and informed family discussions

✔️ A steady rhythm of support to stay organized, prepared, and connected
