The 12-Month Comprehensive Life Enrichment Plan is ideal for seniors and families looking for year-long, full-spectrum planning. This premium package offers personalized guidance through all stages of aging—designed to reduce stress, plan ahead, and make day-to-day life more manageable.

It begins with a 120-minute planning session (in-person or virtual), followed by a 90- minute review session to walk through the proposed recommendations, clarify priorities, and refine your support roadmap. Whether you're preparing for future needs or actively managing current changes, this plan offers complete peace of mind.

What's Included:

✔️ 120-minute personalized planning session (in-person or virtual)

✔️ 90-minute review session to discuss the suggested support (in-person or virtual)

✔️ Follow up every other month for 12 months ( 6 appointments total)

✔️ End-of-life planning discussion

✔️ Up to 10 possible areas of concern (See below)

✔️ A fully customizable plan—only including the areas most relevant to you

✔️ Personal care services

✔️ Guidance around meal services, nutrition supports, or delivery options

✔️ Transitioning to alternative housing or managing current housing (Can be combined with other areas of concern)

✔️ Home maintenance and safety planning

✔️ Financial Budgeting

✔️ Coordination of travel and seasonal plans

✔️ Life Enrichment Activities (e.g., social outing, hobby, or wellness check-in)

✔️ Support focused on fitness and mobility for long-term independence

✔️ Help navigating urgent or unexpected challenges

✔️ Transportation planning, including:

• • Assistance with loss of driver's license

  • Support with selling a vehicle

  • Guidance on available local transportation option

If you’d like structured ongoing check-ins, you can add Monthly, Biweekly, Weekly, or Weekday Support at any time. These support plans are billed on a recurring basis and can be paused or canceled as your needs change.
