3 Month Personalized Plan

The 3-Month Life Enrichment Plan is a personalized support package designed to help seniors and their families confidently navigate the aging journey within the next 3 months. Whether you're focusing on the short-term plan or responding to current challenges, we’ll work together to build a compassionate and practical roadmap tailored to your unique needs.

The service begins with a 90-minute planning session (in person or via video call), followed by a 1-hour review session to walk through the proposed recommendations, clarify priorities, and refine your support roadmap.

What's Included:

✔️ 90-minute personalized planning session (in-person or virtual)

✔️ 1-hour review session to discuss the suggested support (in-person or virtual)

✔️ Monthly follow up for the 2 months after the review session

✔️ End-of-life planning discussion

✔️ Up to 3 possible areas of concern (See below)

✔️ A fully customizable plan—only including the areas most relevant to you

 

Possible areas of concern:

✔️ Personal care services

✔️ Guidance around meal services, nutrition supports, or delivery options

✔️ Transitioning to alternative housing or managing current housing (Can't be combined with other areas of concern)

✔️ Help navigating urgent or unexpected challenges

✔️ Other immediate concerns (at the discretion of the Epilogue Specialist)

 

Looking for more regular check-ins after your plan is in place? You can add Monthly, Biweekly, Weekly, or Weekday Support at any time. These flexible options are available on a recurring payment model that can be paused or canceled to suit your needs.
