Guiding Path Personalized Plan

The Guiding Path Plan offers an expanded level of care and coordination for seniors and their families. It builds on the foundational services included in the 3-Month Plan—offering support for up to 6 areas of concern.

The package begins with a 120-minute planning session (in-person or virtual), followed by a 60-minute review session to walk through the proposed recommendations, clarify priorities, and refine your support roadmap.

What's Included:

✔️ 120-minute personalized planning session (in-person or virtual)

✔️ 60-minute follow-up review session (in-person or virtual)

✔️ Follow up every other month for 6 months ( 3 appointments total)

✔️ End-of-life planning discussion

✔️ Up to 6 possible areas of concern (See below)

✔️ A fully customizable plan—only including the areas most relevant to you

 

Possible areas of concern:

✔️ Personal care services

✔️ Guidance around meal services, nutrition supports, or delivery options

✔️ Transitioning to alternative housing or managing current housing (Can be combined with 2 other areas of concern)

✔️ Home maintenance and safety planning

✔️ One recurring activity of your choice (e.g., social outing, hobby, or wellness check-in)

✔️ Support focused on fitness and mobility for long-term independence

✔️ Help navigating urgent or unexpected challenges

✔️ Transportation planning, including:

• • Assistance with loss of driver's license

  • Support with selling a vehicle

  • Guidance on available local transportation options
    

✔️ Other immediate concerns (at the discretion of the Epilogue Specialists)

 

If you’d like continued hands-on support, you can add Monthly, Biweekly, Weekly, or Weekday Support. These optional add-ons can be paused or canceled any time, offering flexibility as your situation changes.
